Job summary

Oak Street Health in Indianapolis is hiring a Nurse Practitioner to provide value-based primary care for adults on Medicare. You will work with a collaborative care team to deliver longitudinal care, educate patients, and coordinate with physicians and specialists.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program required.
• Active, non-probationary NP license in a US state.
• Active DEA license.
• US Work Authorization.
• National certification in Family NP, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP.

Responsibilities

• Provide effective and equitable value-based primary care to adults on Medicare.
• See patients independently and collaborate with physicians as needed.
• Perform Annual wellness visits and health risk assessments with thorough documentation.
• Coordinate care with providers, facilities and care managers.
• Lead population health initiatives in coordination with the care team.
• Educate patients on health conditions and care plans.
