Qualifications

• MSN, DNP, or NP certificate graduate required.
• National certification in at least one NP specialty required (FNP, AGPCNP, ANP, GNP).
• Active state NP license and DEA license required.

Responsibilities

• Provide effective primary care to adults on Medicare.
• Conduct office visits for routine and acute issues.
• Perform annual wellness visits and health risk assessments with thorough documentation.
• Coordinate care with other providers and facilities.
• Lead population health initiatives within the care team.
• Assist with triage and patient outreach by phone.
• Educate patients on conditions and care plans.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
