Job summary

Oak Street Health in Philadelphia seeks a Nurse Practitioner to deliver value-based primary care for adults on Medicare within an outpatient clinic. You will practice independently within a collaborative team of physicians, NPs, PAs, and care managers to ensure comprehensive, patient-centered care.

The role emphasizes care coordination, wellness visits, and population health, with a strong focus on building lasting patient relationships and improving health outcomes in a supportive network.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification in Family NP, Adult-Gerontology PCP, Adult NP, or Gerontological NP
• Active, non-probationary state NP license
• Active DEA license
• US Work Authorization

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with providers, specialists, facilities and agencies.
• Population health leadership with the Care Team ensuring evidence-based screenings.
• Assist with phone triage and outreach.
• Educate patients on health conditions, care plans and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Other duties as assigned.
