Job summary

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• Active, non-probationary state NP license.
• Active DEA license.
• US Work Authorization.

Responsibilities

• Provide effective, value-based primary care to adults on Medicare.
• See patients independently within a collaborative care team.
• Perform annual wellness visits and health risk assessments.
• Coordinate care with physicians, specialists, and care managers.
• Lead population health initiatives and ensure appropriate screenings.
• Educate patients on health conditions, care plans and treatments.
• Contribute to clinic operations and participate in promotional activities.
