Upperline Health Inc is seeking a Practice Success Partner to guide partner practices through Value Based Care enrollment, ensuring understanding of processes and technology. The role involves onboarding clinics, analyzing enrollment data, and maintaining strong partner relationships to support patients throughout their participation.

Travel up to 50% is required to partner practices along the East Coast to Midwest; remote applicants in designated states may apply.

Qualifikationen

• High school diploma or equivalent; associate's or bachelor’s degree preferred.
• Previous experience in healthcare is required; Knowledge of value-based care programs is preferred.
• Previous experience deploying programs with a technology component.
• Excellent communication and interpersonal skills, with the ability to build rapport quickly across all areas of clinic staff engage through story-telling
• Demonstrated ability to provide training in an effective manner
• Demonstrated ability navigating conflictStrong organizational and time management skills.
• Proficiency in computer systems and data entry.
• Ability to work independently and as part of a team.

Aufgaben

• Onboarding: Guide partner practices through the Value Based Care program enrollment process, ensuring understanding of enrollment process, required documentation, applicable technology, forms and compliant processes.
• Data tracking and performance mitigation: Analyze enrollment data to identify trends and opportunities for improvement; work with clinics on root causes and mitigation plans.
• Partner engagement: Build strong relationships with partner clinics and provide ongoing support to patients via the program.
• Collaboration: Work with care teams and stakeholders to ensure program success.
• Quality improvement: Contribute to process improvements to enhance enrollment efficiency and communicate opportunities to leadership.
