Job summary

Upperline Health is seeking a Practice Success Partner to guide partner practices through Value Based Care program enrollment, ensuring understanding of processes, documentation, and technology. You will onboard clinics, train staff, and ensure successful milestones.

Role requires travel up to 50% and remote-ready applicants in AL, FL, GA, IL, IN, KY, MD, MI, NV, PA, SC, TN, TX, and DC. Strong communication and organizational skills are essential.

Qualifications

• Experience in healthcare settings is required.
• Familiarity with value-based care programs preferred.
• Experience deploying programs with a technology component.
• Excellent communication and interpersonal skills; ability to engage diverse clinic staff.

Responsibilities

• Onboard partner practices through Value Based Care enrollment and provide training on processes, docs, and technology.
• Track enrollment data, identify trends, and work with clinics on root causes and mitigation plans.
• Build strong relationships with partner clinics and provide ongoing support to patients.
