Job summary

Moms In Motion is hiring a Service Facilitator to connect families with Virginia Medicaid Waiver programs and ensure safe, healthy, independent living. You will travel locally to client homes and work remotely from a laptop/tablet, guiding care planning and problem-solving across authorizations and timesheets.

The role requires at least 2 years of relevant experience; no degree is required. A valid driver’s license and reliable vehicle are essential. Training and benefits are provided.

Qualifications

• 2+ years experience helping individuals with disabilities or elderly.
• No degree required; focus on heart and experience.
• Valid driver’s license and reliable vehicle required.
• Tech-savvy enough for email, documents, spreadsheets and video calls.

Responsibilities

• Visit clients in their homes on local routes.
• Guide families through Virginia Medicaid Waiver programs.
• Draft care plans and assessments that drive outcomes.
• Resolve issues with authorizations, timesheets and related tasks.
• Build trusting relationships with families while working remotely from a laptop/tablet.
