Job summary

Moms In Motion is hiring Service Facilitators to visit families in VA, guiding them through Medicaid Waiver programs and coordinating care to help clients live safe, healthy, independent lives. You will travel locally to client homes and work remotely from a laptop/tablet when needed.

We value compassion, organization, and community focus, with 2+ years supporting individuals with disabilities or the elderly and no degree required. Driving is required.

Qualifications

• Experience supporting individuals with disabilities or the elderly.
• No degree required; relevant heart and experience welcomed.
• A valid driver’s license and reliable vehicle.
• Ability to pass a background check and provide two professional references.

Responsibilities

• Visit clients in their homes across local areas.
• Serve as the guide for families navigating VA waiver programs.
• Draft plans of care and assessments that drive meaningful outcomes.
• Troubleshoot issues like authorizations and timesheets.
• Build lasting, respectful relationships with families.
• Work remotely from a laptop/tablet as needed.
