Moms In Motion is seeking a Service Facilitator to visit clients in their homes across Virginia, guiding families through Medicaid Waiver programs and ensuring safe, healthy, independent living. The role emphasizes empathy, organization, and community impact.

Applicants need at least 2 years in disability or elder support; no degree is required. A valid driver’s license and reliable vehicle are needed, with local daily travel and occasional remote work from a laptop/tablet.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience valued over credentials.
• Tech-savvy with email, documents, spreadsheets, portal navigation and video calls.
• Valid driver’s license and reliable vehicle (no client transport).

Responsibilities

• Hit the road to visit clients in their homes.
• Be the go-to guide for families navigating waiver programs.
• Write up plans of care and assessments that actually make a difference.
• Troubleshoot issues like service authorizations and timesheets.
• Build lasting relationships with families based on respect, patience, and trust.
• Work remotely from your laptop/tablet.
