Moms In Motion is seeking a Service Facilitator (SF) Case Manager in Virginia. You will visit client homes locally, help families navigate Medicaid waiver programs (CCC+, CL & FIS, EPSDT), and develop plans of care to support safe, healthy, independent lives.

No degree required; we value heart and experience. Must have a valid driver's license and be comfortable using email, documents, spreadsheets, and video calls; pass background check and provide two references; remote work from your

Qualifications

• 2+ years experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience matter more.
• Valid driver's license and reliable vehicle.
• Proficient in email, documents, spreadsheets, portal navigation and video calls.
• Able to pass a background check and provide 2 professional references.
• Bonus: Person-Centered Thinking/Planning training.

Responsibilities

• Visit clients in their homes locally.
• Guide families navigating waiver programs.
• Write care plans and assessments that make a difference.
• Troubleshoot service authorizations, timesheets, and more.
• Build lasting relationships with families based on respect, patience, and trust.
