Job summary

Moms In Motion seeks a Service Facilitator to connect with families across Virginia, helping them navigate Medicaid Waiver programs and ensuring safe, healthy, independent living. You’ll hit the road to visit clients in their homes and work from your own laptop.

We value compassion, organization, and clear communication. Requirements include at least 2 years of experience in disability or elder care, a valid driver’s license, and the ability to pass a background check. Training provided.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience matter.
• Must have a valid driver's license and reliable vehicle.
• Excellent written and spoken communication.
• Able to pass a background check and provide 2 professional references.

Responsibilities

• Hit the road (locally!) to visit clients in their homes.
• Be the go-to guide for families navigating waiver programs.
• Write up plans of care and assessments that actually make a difference.
• Troubleshoot challenges like service authorizations and timesheets.
• Build lasting relationships with families built on respect, patience and trust.
• Work remotely from your laptop/tablet.
