Job summary

Vail Health Hospital in the United States seeks a Triage RN to perform care coordination and triage nursing, assessing patient needs and guiding dispositions to emergency, urgent or clinic care. You will support providers with triaging calls and messages and document thoroughly in the EHR.

Join a care coordination team focused on quality outcomes, participate in KPI review huddles, and contribute to CPC+ processes. Colorado-licensed or compact-state RNs preferred; 3 years' experience helpful.

Qualifications

• Three years of clinical experience preferred with patient care and admin duties.
• Nursing assessment skills with basic theory; HIPAA compliance required.
• Colorado RN or compact state license required.

Responsibilities

• Assess patient health needs using triage protocols and provide disposition decisions.
• Support providers by triaging patient calls and portal messages.
• Document symptoms, assessments, plans and follow-up in the EHR.
• Participate in weekly huddles to review KPI and drive care coordination.
• Contribute to CPC+ care coordination and performance improvement.
