Job summary

Vail Health is seeking a Triage Registered Nurse to perform care coordination and triage functions in a dynamic health system. You will assess patient needs, triage calls and portal messages, and support providers within a care coordination team.

Responsibilities include documenting assessments, following policies, and contributing to quality improvement initiatives. Requires an RN license and 3+ years of experience; Colorado license or compact state license is needed.

Qualifications

• 3+ years clinical experience with patient care and admin duties preferred.
• Nursing assessment skills with/without visual inspection.
• Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules.
• State of Colorado RN or valid compact state license required.

Responsibilities

• Triage RN performs care coordination and nursing functions to assess patient needs using protocols.
• Supports providers in triaging patient calls and portal messages.
• Part of care coordination/clinical team supporting general care processes.
• Documents symptoms/complaints, nursing assessment, advice and response.
• Participates in weekly huddles to review KPI and obtain provider input for care actions.
• Follows policies to improve health care outcomes and access to care.
• Assists in quality improvement and CPC+ Care Coordination processes.
• Ensures pertinent medical information is forwarded for continuity of care.
• Documents in EHR the triage assessment, plan, implementation and follow-up.
