Job summary

Vail Health is hiring a Triage RN to perform care coordination and professional nursing functions, assessing patient health needs and triaging dispositions to emergency care, urgent care, and clinic appointments within licensed practice. You will support providers in triaging calls and messages as part of a care coordination team.

The role requires 3 years of clinical experience, knowledge of disease processes, nursing assessment skills, and compliance with HIPAA/OSHA.

Qualifications

• 3 years clinical experience with applicable patient care and administrative duties preferred.
• Knowledge of disease processes, normal growth and development of all age groups required.
• Nursing assessment skills with/without visual inspection utilizing basic nursing theory required.
• Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules and regulations required.

Responsibilities

• Evaluates and assesses patient health care needs using triage protocols.
• Participates in weekly huddles to review KPI and ensure KPI-aligned workflows.
• Obtains input from the provider to determine best clinical course.
• Documents symptoms, nursing assessment, advice provided and patient/caller response.
• Follows policies and protocols to improve health care outcomes and access.
• Coordinate and schedule same-day and future appointments.
• Manage prescription refills per protocol.
• Provide patient education on chronic disease and preventive care.
