Vail Health is seeking a Triage RN to support care coordination and professional nursing functions, assessing patient health concerns via established protocols to direct dispositions to emergency care, urgent care, or clinic appointments. You will assist in triaging calls and portal messages, supporting the clinical team, and documenting care in the EHR.

The role requires 3 years of clinical experience and a Colorado RN license.

Qualificações

• 3 years clinical experience with patient care and administrative duties preferred.
• Knowledge of disease processes and normal growth and development for all age groups.
• Nursing assessment skills with/without visual inspection using basic nursing theory.
• Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules and regulations.

Responsabilidades

• Evaluates and assesses patient health care needs using triage protocols.
• Participates in weekly huddles to review KPI and ensure KPI-approved workflows.
• Obtains input from providers to determine clinical course of action.
• Documents symptoms/complaints, nursing assessment, advice provided and patient/caller response.
• Follows policies and protocols to improve health care outcomes and access.
• Assists in quality improvement and in coordination of same-day and future appointments.
• Manage prescriptions refills per protocol and participate in CPC+ Care Coordination.
• Provide patient education on chronic disease, prevention and family health.
• Documents in EHR triage assessment, plan, implementation and follow-up.
