Vail Health Hospital in Madison, WI, seeks a Triage Registered Nurse to provide care coordination and triage for emergency, urgent care, and clinic referrals. You will support providers, triage calls and portal messages, and document assessments in the EHR.

The role emphasizes collaboration with care teams, adherence to protocols, and improving health outcomes. Requires 3 years clinical experience and Colorado license or valid compact state license.

Qualifications

• 3 years clinical experience with patient care and admin duties preferred.
• Knowledge of disease processes and growth/development for all ages.
• Nursing assessment skills with or without visual inspection.
• Knowledge of medical terminology, equipment, HIPAA compliance and OSHA rules.
• State Colorado RN license or valid compact state license required.

Responsibilities

• Evaluates and assesses patient needs using triage protocols.
• Participates in weekly huddles to review KPI and KPI-related workflows.
• Obtains input from provider to determine best clinical actions.
• Documents symptoms, nursing assessment, advice, and patient responses in the EHR.
• Coordinate and schedule same-day and future appointments on provider schedules.
• Manage prescription refills per protocol.
• Provide patient education on chronic disease, prevention and family health.
• Adhere to Just Culture and organizational values.
