Vail Health Hospital is seeking a Triage RN to perform care coordination and triage nursing functions. You will assess patient health concerns using protocols and support providers with triaged calls and portal messages.

The role requires 3 years of clinical experience, strong nursing assessment skills, and knowledge of HIPAA/OSHA regulations. Eligible candidates should hold CO or compact state RN licensure and be prepared to work within the Colorado region.

Qualificações

• 3 years clinical experience with patient care and administrative duties preferred.
• Knowledge of disease processes and growth/development across age groups.
• Nursing assessment skills with/without visual inspection.
• HIPAA compliance and OSHA rules knowledge required.

Responsabilidades

• Evaluates and assesses patients' health care needs using triage protocols and guidelines.
• Participates in weekly huddles to review KPI and ensure KPI adherence.
• Obtains input from providers to determine best clinical actions.
• Documents symptoms, nursing assessment, advice and responses in EHR.
• Follow policies and protocols to improve care access and outcomes.
• Assist in quality improvement initiatives as requested by leadership.
• Coordinate same-day and future provider appointments.
• Manage prescriptions refills per protocol.
• Provide patient education on chronic disease and preventive care.
