Vail Health Hospital in the United States is seeking a Triage Registered Nurse to perform care coordination and triage nursing duties. You will assess patient needs using protocols and support providers through triage calls and portal messages.

Responsibilities include documenting assessments, coordinating appointments, managing referrals, and providing patient education while upholding HIPAA and OSHA standards. This role emphasizes teamwork within a care coordination/clinical unit.

Qualifications

• 3 years clinical experience with applicable patient care and administrative duties preferred.
• Knowledge of disease processes, normal growth and development of all age groups required.
• Nursing assessment skills with/without visual inspection utilizing basic nursing theory required.
• Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules and regulations required.

Responsibilities

• Evaluates and assesses patient health care needs using triage protocols and guidelines.
• Participates in weekly huddles to review KPI and ensure workflows meet KPI expectations.
• Obtains input from providers to determine best clinical course of action.
• Documents symptoms, nursing assessments, advice provided and patient/caller responses.
• Follows policies and protocols to improve health care outcomes and access to care.
• Coordinate and schedule same-day and future appointments on provider schedules.
• Manage prescription refills per protocol.
• Provide patient education on chronic disease, preventive care and family health.
• Participates in CPC+ Care Coordination process and performance improvement activities.
