Vail Health Hospital is seeking a Triage RN to perform care coordination and triage nursing duties, assessing patient health concerns using established protocols. You will support providers through triaging calls and portal messages, and assist in routing patients to emergency care, urgent care, or clinics as appropriate.

Bring at least 3 years of clinical nursing experience, strong assessment skills, and knowledge of HIPAA and OSHA regulations.

Qualificações

• 3 years clinical nursing experience preferred.
• Knowledge of disease processes across age groups.
• Nursing assessment skills with/without visual inspection.

Responsabilidades

• Evaluates patient health care needs using triage protocols.
• Participates in weekly huddles to review KPI.
• Obtains input from provider to determine clinical course of action.
• Documents symptoms, assessment, advice, and patient response in EHR.
• Follows policies and protocols to improve outcomes and access to care.
• Assists in quality improvement initiatives as requested by leadership.
• Coordinate and schedule same-day and future appointments.
• Manage prescription refills per protocol.
• Provide patient education on chronic disease and prevention.
• Support CPC+ Care Coordination team processes.
• Ensure relevant medical information is forwarded to proper departments.
