Job summary

Life Care Centers of America is seeking an RN MDS Coordinator who travels to skilled nursing facilities to complete and submit timely MDS assessments, CCAs, and care plans in accordance with CMS RAI Manual regulations.

The role requires an active RN license, a relevant nursing degree, and at least two years of nursing experience, preferably in geriatric care. CPR certification is required at hire and must remain current.

Qualifications

• Active RN license in good standing.
• Two years nursing experience; geriatric preferred.
• Associate or Bachelor degree in nursing.
• CPR certification required.

Responsibilities

• Coordinate and submit MDS Assessments, CCAs, and Care Plans per CMS regulations.
• Report patient condition changes to the DON.
• Educate direct care staff on CMS RAI changes impacting documentation.
• Review Interdisciplinary Care Plans and Final Validation Reports.
• Attend PPS meetings and other required meetings; provide staff nurse duties as needed.
• Maintain confidentiality and professional conduct.
