Job summary

Life Care Centers of America seeks an RN MDS Coordinator to oversee the accurate completion and submission of MDS assessments, CCAs, and care plans in accordance with CMS regulations within a post-acute care setting.

The role requires an RN license, a nursing degree, and at least 2 years of nursing experience, preferably in geriatrics. CPR must be current. Apply to join our patient-centered team in a regulated environment.

Qualifications

• RN license in good standing in the applicable state.
• Bachelor's or Associate degree in nursing required.
• Minimum 2 years of nursing experience; geriatric experience preferred.
• CPR certification upon hire or during orientation.

Responsibilities

• Coordinate and assist with MDS Assessments, CCAs, and Care Plans.
• Report changes in a patient’s condition to the DON.
• Educate staff on CMS RAI updates and regulations that impact documentation.
• Review Final Validation Reports and ensure CMS QIES billing readiness.
• Attend PPS meetings and other required meetings for compliance.
• Provide patient-centered care with excellent service and teamwork.
• Maintain confidentiality and professional conduct.
