Job summary

Life Care Centers of America seeks a Travel MDS Coordinator (RN) to support multiple facilities across Indiana. You will coordinate and assist with accurate MDS Assessments, CCAs, and Care Plans under CMS guidelines, ensuring timely submissions and compliance.

The role requires an active RN license, 2 years of nursing experience (geriatric preferred), and CPR certification. You will collaborate with interdisciplinary teams and participate in daily PPS meetings while providing high-quality

Qualifications

RN license required and in good standing
Two years nursing experience, geriatric preferred
CPR certification on hire or during orientation

Responsibilities

Coordinate and assist with completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans according to CMS RAI Manual Regulations
Report changes in patient condition identified by MDS Assessment to the DON
Educate direct care staff on CMS RAI updates impacting documentation
Review Interdisciplinary Comprehensive Care Plans and Final Validation Reports
Attend Daily PPS Meeting and other meetings as requested
Perform staff nurse functions as required
Demonstrate excellent customer service and professional conduct
