Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations across a defined coverage area. The role involves traveling up to 100%, home-based visits, and coordinating with health plans to bridge care gaps while maintaining safety and quality standards.

The visit lasts about 45 minutes and includes medication and medical history review, a physical evaluation, and optional diagnostic tests as ordered by the health plan. MD/DO licenses required.

Responsabilidades
