Job summary

Signify Health, part of CVS Health®, is seeking a Full Time Physician to provide in-home health evaluations. You will travel across a defined coverage area, engaging with members in their homes and coordinating care with health plans.

Required are an active MD/DO license and the ability to obtain multi-state licenses. This role emphasizes patient-centered care, collaboration with teams, and flexibility for travel and virtual visits.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area, as required.

Responsibilities

• Provide in-home health evaluations and build trusted relationships with patients.
• Travel across defined coverage area to perform in-home visits (up to 100%).
• Use iPad and clinical workflow to document visits and data.
• Conduct virtual visits as needed to support care.
• Deliver diagnostic and preventive services as indicated by health plans.
• Travel locally and within licensed states; collaborate with care teams.
