Signify Health, part of CVS Health, is seeking a Full-Time Physician to perform in-home health evaluations across a defined coverage area. The role involves travel, home visits, and coordinating with health plans to bridge care gaps.

You will conduct 45-minute visits, review meds and history, perform examinations, and order tests only when directed by the health plan. A valid MD/DO license is required across relevant states.

Competenze

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses as required.
• In-home clinician delivering health evaluations to health plan members.
