Job summary

Signify Health, part of CVS Health®, is seeking a Full Time Physician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. You’ll travel across a defined coverage area, conduct 45‑minute visits, review medications and medical history, and perform a physical evaluation as needed.

You’ll collaborate with health plans to connect members with the right care, and may conduct virtual visits.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses as required.
• MDs and DOs only.

Responsibilities

• Provide in-home health evaluations to patients.
• Travel across defined coverage area (up to 100%).
• Review medical history and perform physical evaluation.
• Coordinate with health plans to bridge gaps in care.
