Job summary

Signify Health, part of CVS Health®, is seeking a Full Time Physician to provide In-Home Health Evaluations across a defined coverage area. The role involves visiting patients in their homes, traveling locally and nationally as needed, and coordinating with health plans and care teams.

The visit lasts about 45 minutes and includes med history review, physical evaluation, and potential diagnostic tests. Applicants must hold active, unrestricted MD/DO licenses and be willing to travel as required.

Qualifications

• Active, unrestricted medical license in coverage area.
• Ability to obtain multi-state licenses as required.
• Experience with in-home health evaluations preferred.

Responsibilities

• Perform in-home health evaluations for patients.
• Conduct 45-minute visits including history, meds, and physicals.
• Coordinate with health plans and care teams; travel as needed.
• Utilize iPad-based workflow; implement diagnostic tests if ordered.
