Job summary

Signify Health, a CVS Health company, is hiring a Full-Time Physician to perform in-home health evaluations across a defined coverage area. Visits are typically 45 minutes with health history review, physical exam, and optional diagnostic tests. Travel up to 100% may be required, including potential overnight stays.

You will work with a national network of clinicians to bring personalized care directly to patients’ homes. Licensing in multiple states and a valid unrestricted license are required.

Qualifications

• Active, unrestricted medical license in coverage area(s).
• Able to obtain and maintain multi-state licenses as needed.

Responsibilities

• Bring your heart to every visit and join a national network of clinicians dedicated to improving health outcomes.
• Visit millions of people nationwide to connect them with the right care for their unique needs.
• Collaborate with Signify Health as a clinician, fulfilling the role requirements.
• Conduct virtual visits as needed.
• Provide diagnostic and preventive services (e.g., spirometry, DEE, bone density) as indicated.
• Be flexible to travel locally and within licensed states.
