Job summary

Signify Health, part of CVS Health, seeks a Full Time Physician to provide in-home health evaluations across a defined coverage area. The primary workplace is patients' homes, with travel up to 100%.

You will use an iPad workflow, review medical history, perform physical evaluations, and coordinate with health plans. MD or DO, multi-state licensing required; travel may include overnight stays.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area.

Responsibilities

• Build trusted relationships to improve health outcomes.
• Visit patients in home settings to collect health data and provide evaluations.
• Conduct home visits using iPad workflow, performing medical history review and physical evaluation.
• Conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services as indicated by health plan.
