Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations across defined coverage areas. You will travel up to 100% of the time, conduct visits in patients' homes, review medical histories, perform exams, and coordinate with health plans to bridge gaps in care.

Licensure in multiple states may be required. You’ll use an iPad and a clinical workflow, may conduct additional diagnostics, and be supported by a comprehensive benefits package.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area, as required.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
• Be flexible to travel locally and within licensed states.
