Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area, as required.

Responsibilities

• Provide in-home health evaluations and bridge gaps in care.
• Travel up to 100% of the time across defined coverage area.
• Perform diagnostic tests as ordered by the health plan.
• Use an iPad and clinical workflow during evaluations.
• Complete visits with documentation and appropriate referrals when needed.
