Job summary

CVS Health, through Signify Health, seeks a full-time physician to provide in-home health evaluations across a defined coverage area. You will engage with members in their homes, perform comprehensive health assessments, and use a clinical workflow on an iPad.

Travel up to 100% may be required, with overnight stays as needed. MDs and DOs must hold active licenses in coverage areas and be able to obtain multi-state licenses.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area, as required.

Responsibilities

• Conduct in-home health evaluations and assessments.
• Review medication and medical history; perform physical evaluation.
• Utilize iPad and clinical workflow for each evaluation.
• Provide Signify Health services such as Diagnostic & Preventive Tests as indicated.
• Travel locally and within licensed states as needed.
• Coordinate with health plan and care team to bridge gaps in care.
