Job summary

CVS Health, in collaboration with Signify Health, seeks physicians to perform in-home health evaluations across defined coverage areas. You will engage with members, review medical history, perform physical assessments, and tailor care plans to bridge gaps in care.

Qualifications

• Active, unrestricted licenses in coverage areas.
• Able to obtain and maintain multi-state licenses as required.
• MD or DO credentials preferred.

Responsibilities

• Provide in-home health evaluations with history review and physical examination.
• Travel up to 100% across defined coverage areas.
• Use an iPad with clinical workflow; no prescribing or lab orders required.
• Coordinate with health plans and care teams to connect members with services.
