Job summary

CVS Health Corporation, through Signify Health, is seeking a Full-Time Physician to provide in-home health evaluations across a nationwide coverage area. You will travel up to 100% of the time, conducting visits that include medical history review, a physical evaluation, and coordination with health plans.

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area, as required.

Responsibilities

• Perform in-home health evaluations for patients.
• Travel across defined coverage area as needed.
• Review medication and medical history during visits.
• Use iPad-based clinical workflow; no prescription changes required.
• Coordinate with health plan for diagnostic services as indicated.
