Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations across a defined coverage area. You will travel up to 100% of the time, perform comprehensive assessments using an iPad, and coordinate care with health plans.

Active MD/DO licenses are required, with ability to obtain multi-state licenses. The role offers full-time hours and a comprehensive benefits package.

Qualifications

• Active, unrestricted MD/DO license in the coverage area.
• Able to obtain and maintain multi-state licenses as required.

Responsibilities

• Travel up to 100% to defined coverage area per business needs.
• Conduct in-home health evaluations using iPad and clinical workflow.
• Perform medication and medical history review, physical evaluation, and ordered Diagnostic tests when applicable.
• Provide Diagnostic & Preventive Services as indicated (e.g., spirometry, etc.).
• Be flexible to travel locally and within licensed states.
