VSHP Volunteer State Health Plan, Inc. seeks a dedicated Support Coordinator to empower individuals with I/DD in Sumner County. You will help members live independently, access employment, and coordinate comprehensive services through team collaboration.

Responsibilities include intake, assessments, and creating Person-Centered Plans while traveling to members’ homes. Must have a valid driver’s license and 2+ years in healthcare operations. OT may be required.

Formación

• RN with current certification (CDDN for RNs or DDC for LPNs) preferred
• B.S. in social work, nursing, education, or related field; LCSW/LMFT preferred
• QDDP/QIDP eligibility per federal requirements
• 5+ years as independent SC or case manager for 1915(c) HCBS Waiver with related trainings

Responsabilidades

• Partner with members and families to identify supports and services
• Collaborate with clinical and social support team to meet needs
• Conduct face-to-face visits to assess status and needs
• Authorize home and community based services as appropriate
• Maintain a valid driver's license and comply with safety policies
• Complete required immunizations or tests as needed
• Perform digital literacy assessments
