Job summary

Care Lync is seeking a dedicated Support Coordinator / Service Coordinator / Case Manager to join our team in Summerville, SC. You will develop individualized care plans, coordinate services, and monitor outcomes for adults with IDD, autism, and physical disabilities under the Medicaid Waiver program.

The role requires at least a BS/BA, 3 years of related experience, proficiency with Microsoft Office and state databases, and a valid driver’s license. Travel up to 50% is required.

Qualifications

• BS/BA degree in related field.
• Minimum 3 years case management experience.
• Experience with Microsoft Office and state databases.
• Maintain CPR/First Aid certification and state clearances.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop and modify Person-Centered Service Plans annually or with changes.
• Coordinate with providers to ensure informed participant choices.
• Align ISP with Behavioral and Crisis Plans when applicable.
• Report incidents of abuse, neglect or exploitation promptly.
• Coordinate with other care coordinators to ensure seamless services.
