Job summary

VSHP Volunteer State Health Plan, Inc is seeking a compassionate Care Coordinator to visit members at home or in the community, coordinating services to improve health and independence. You will work with a team to assess needs across physical, behavioral, social, and employment dimensions, and ensure required services are authorized and delivered.

The role requires a TN-licensed RN or active LCSW/LMSW/LAPSW, 3 years clinical experience, and residency within Shelby County, TN.

Qualifications

• PC skills with basic Office and email usage.
• Strong time management to handle multiple care needs.
• Excellent oral and written communication.
• Strong interpersonal and organizational abilities.

Responsibilities

• Partner with members and families to identify supports and services for health goals.
• Collaborate with clinical and social support teams to meet health needs.
• Conduct face-to-face visits to assess status in physical, behavioral, and social aspects.
• Authorize appropriate home and community services and ensure they are provided.
• Maintain valid driver’s license.
• Be prepared for field-based visits and related immunizations/testing if required.
