Job summary

Volunteers of America Mid-States in Memphis, TN is seeking a Home Coordinator to oversee daily operations of supported living homes for adults with IDD. You will supervise Direct Support Professionals and ensure regulatory compliance, care planning, and documentation.

The role requires at least 2 years of IDD experience, strong planning and communication skills, HIPAA confidentiality, and teamwork. On-call availability is expected to support a 24/7 program.

Qualifications

• At least 2 years of experience working with adults who have intellectual and developmental disabilities.
• Knowledge of applicable regulatory requirements.
• Planning and organizational skills.
• Skills in good judgement, supervising, and training staff.
• Ability to communicate effectively in a courteous and professional manner.
• Ability to maintain confidentiality and uphold HIPAA.
• Ability to work as a team member.

Responsibilities

• Maintain safety and cleanliness of the home, while following VOA policies and procedures.
• Supervise and assist consumers in maintaining wholesome and productive lives with emphasis on opportunities for consumers to achieve their goals.
• Train Direct Support Staff per mandated curriculum as required by state and VOA policies and regulations.
• Accompany consumers on scheduled and emergency medical appointments and communicate with the agency’s Registered Nurse.
• Attend Interdisciplinary team meetings as assigned by the Residential Service Coordinator.
• Complete weekly, monthly, and quarterly documentation to assure compliance with PCISP goals and State Regulations.
• Train staff on each client’s Individualized Support Plan and complete annual training courses.
• Plan and coordinate leisure, recreational, and social activities.
• Assist consumers in maintaining relationships with family and friends.
• Supervise staff and assigned customers; be available on an on-call basis.
• Develop staff schedules within budgetary constraints to meet consumer needs and approved cost plans.
• Review hours and timesheets; participate in overtime evaluations.
• Monitor and evaluate staff performance; recommend disciplinary action as needed.
• Performance and quality improvement duties as assigned by the Residential Service Coordinator.
• Initiate purchase requests to meet consumer needs.
• Ensure grocery/supply shopping is done appropriately within budgets.
• Maintain vehicle maintenance.
• Ensure MARS accuracy and updates.
• Communicate health concerns to the Registered Nurse.
• Document medical appointments in the Practical Health System.
