Job summary

VSHP Volunteer State Health Plan, Inc. is seeking a dedicated Support Coordinator to empower individuals with I/DD in Hamilton County, TN. You will assess needs, develop person-centered plans, and coordinate home and community-based services while traveling to members’ homes.

The role requires strong collaboration with clinical and social support teams and a current driver's license. Ideal candidates have RN with related certification or a social work/nursing degree plus HCBS / QDDP/QIDP

Qualifications

• RN with advanced certification (CDDN/QDDP) preferred.
• B.S. in social work, nursing, education, human service or health care related field; MSW/LCSW preferred.
• QDDP or QIDP qualifications acceptable.
• 5+ years as independent Support Coordinator or HCBS case manager preferred.

Responsibilities

• Partner with members and families to identify needed supports and direct services.
• Collaborate with clinical and social support teams to meet physical, behavioral health and long-term service needs.
• Conduct thorough face-to-face visits and assess member status and needs across domains.
• Authorize appropriate home and community-based services and confirm services are provided.
