American Traveler is seeking a Home Health RN to manage patient caseloads in the community setting in Vermont. A VT or compact RN license and at least 2 years of nursing experience are required.

The role involves case management, a M-F 8am-4pm schedule with some weekends, on-call and holiday coverage, and comprehensive patient assessments to develop individualized care plans. Collaboration with the care team ensures safe outcomes and proper documentation.

Formación

• Active VT or compact RN license required upon acceptance.
• Minimum of 2 years of nursing experience required.
• Experience with high-tech skills such as IV therapy, wound VAC, and complex wound care.
• Familiarity with OASIS documentation preferred.
• Experience with McKesson charting system preferred.

Responsabilidades

• Home health setting providing care to patients in their residences.
• Serves as Case Manager for assigned patients, managing caseloads and visit times per agency productivity standards.
• M-F schedule from 8am-4pm with some weekend work required.
• On-call responsibilities required, including hospice coverage during on-call periods.
• Holiday coverage required.
• Conduct comprehensive patient and caregiver assessments using the nursing process to develop individualized care plans.
• Implement medical plans of care, nursing interventions, and procedures to ensure safe and effective patient outcomes.
• Collaborate with LPNs, delegating tasks and reviewing documentation to ensure continuity of care.
• Provide patient and caregiver education on medications, treatment options, discharge instructions, and reportable conditions.
• Coordinate interdisciplinary care with physicians, providers, and other team members to support the plan of care.
• Complete required agency, state, and federal forms in a timely manner.
• Participate in monthly peer review audits and case conferences with the care team.
• Coordinate safe patient discharges and transfers in accordance with regulations.
• Maintain accurate and up-to-date nursing documentation to support continuity of care.
