American Traveler is hiring a Home Health RN with at least 1 year of experience and an active VT or compact RN license for a full-time contract position with a community-focused home health and hospice provider.

Responsibilities

• Home health and hospice care setting serving individuals of all backgrounds
• Provides individualized, goal-oriented nursing care per physician orders
• Serves as Case Manager when this position is the lead discipline
• Uses Homecare Homebase (HCHB) EMR system
• Monday-Friday day shift schedule with weekend rotation approximately every 4th week
• No holiday coverage required
• 20-week contract duration
• Delivers health promotion, long-term care, and maternal-and-child health services
• Works with a not-for-profit organization focused on community well-being
