Job summary

American Traveler, LLC. is hiring a registered nurse for a Home Health role in Utah, requiring a UT or compact RN license and at least one year of home health, hospice, or acute care experience. Position features four 10‑hour shifts weekly with alternating weekends and requires driving to the office to transfer patient records.

Epic charting is used to coordinate patient care. Candidates should have an ASN or Nursing diploma, current BLS, and a valid driver's license with reliable

Qualifications

• Active UT or compact RN license required.
• Current BLS certification (AHA) that does not expire before start date.
• ASN or Nursing diploma required.
• Minimum 1 year of experience in home health, hospice or acute care preferred.
• Valid driver's license with reliable transportation and acceptable driving record.
• Ability to transfer and assist patients independently, including lifting and navigating stairs.
• Basic computer skills and strong verbal and written communication.

Responsibilities

• Home Health setting serving adult and pediatric patients in their home environment.
• Four 10-hour day shifts per week, Monday, Wednesday, Friday, with every other Saturday and Sunday.
• Requires ability to commute to the office twice daily to electronically transfer patient files.
• Uses Epic for charting.
• Assess, plan, implement, and evaluate individualized care plans for home health and hospice patients.
• Coordinate and collaborate with interdisciplinary care teams, including physicians and support staff.
• Deliver care in compliance with hospice/home health policies, legal and regulatory standards.
• Promote patient and family education and prepare patients for transitions in care.
• RTO must be submitted with candidate profile.
• Returning Intermountain employees must be separated from the facility for at least one year before being eligible for a travel assignment.
