Job summary
Home Care Network, Inc. is seeking a skilled RN Case Manager / Recertification Nurse for a Part-Time or PRN role serving Northwest Ohio.
The position involves case management, SOCs, admissions and discharges, recertifications, and supervisory reviews across multiple counties with no overnight travel required. The role offers flexible scheduling, mileage reimbursement, and opportunities for career advancement within a nurse-owned, locally led home health agency.
Qualifications
- RN with active Ohio license and at least 1 year of home health experience.
- Experience with OASIS and NON-OASIS assessments.
- Strong documentation and staff leadership abilities.
- Willingness to travel within stated radius; valid driver''s license.
- CPR and First Aid certifications or ability to obtain.
Responsibilities
- Coordinate and evaluate continued home health services for patients.
- Document patient status and update care plans.
- Coordinate with physicians and care team; ensure timely communication.
- Supervise RN, LPN, HHA, and aides; provide leadership and training.
- Travel to assigned visits within the regional service area.
