Job summary

Home Care Network, Inc. seeks a Part-Time or PRN RN Case Manager & Recertification Nurse (RNCM) to coordinate and supervise home health care services across Northwest Ohio.

Travel to assigned counties is required, with a focus on ensuring high-quality patient outcomes and regulatory compliance. The role offers a defined leadership path, with supervision of clinical staff, case management of SOC, recertifications, ROC, and admissions, and potential for additional hours based on caseload and

Qualifications

• Minimum of 1 year in case management, admissions/discharges, wound care, PICC/PORT/IV care, transitional care, lab draws, OASIS and Non-OASIS assessments.
• Strong documentation and field staff leadership required.
• Willing to travel within NW Ohio service areas and surrounding counties.

Responsibilities

• Coordinate, re-evaluate and confirm medical necessity of continued home health care services.
• Oversee patient services, treatment plans and discharge planning.
• Direct and supervise RN, LPN, HHA, and PCA teams; ensure timely and compliant communications.
