Job summary

Home Care Network, Inc. is seeking a Part-Time or PRN RN Case Manager to coordinate care across Northwest Ohio, with travel required to multiple counties and up to a 2-hour radius. You will lead field staff, manage SOCs and recertifications, and ensure compliance and quality care for our clients.

You will report to the NW OH Regional Program Administrator and have potential career growth into leadership roles, with flexible schedules and mileage reimbursement to support travel.

Qualifications

• Minimum of 1 year in case management, admissions/discharges, wound care, PICC/PORT/IV care, transitional care, lab draws, and OASIS/non-OASIS assessments.

Responsibilities

• Coordinate patient assessments and care plans in home health settings.
• Direct and supervise nursing staff and caregivers.
• Ensure compliance with state/federal guidelines and agency policies.
