Home Care Network, Inc. in Ohio is seeking a Part-Time or PRN RN Case Manager/Recertification Nurse (RNCM) to lead field teams, manage cases, and coordinate care for clients across Northwest Ohio.

The role emphasizes supervision, documentation, and collaboration with clinicians, payors, and families, with travel required and no overnights. Eligible for bonuses and flexible schedules within a nurse-owned agency.

Qualificações

• Minimum 1 year experience in home health case management and supervision.
• Experience with SOC, admissions, discharges, recertifications and transitions of care.
• Strong documentation and field leadership skills.

Responsabilidades

• Coordinate assessments, plan care and monitor patient status in home settings.
• Supervise a team of clinicians and coordinate care with physicians and families.
• Ensure timely documentation and compliance with state/federal guidelines.
• Travel within NW Ohio counties to serve clients and manage caseloads.
