Home Care Network, Inc. in Northwest Ohio seeks an RN Case Manager/Recertification Nurse (RNCM) for Part-Time or PRN work. The role involves case management, SOCs, admissions and recertifications with travel to multiple counties.

Compensation ranges $36–$40 per hour, with mileage reimbursement, flexible schedules, and potential bonuses. Northern Ohio travel and supervision duties are included in a field leadership capacity.

Qualifications

• RN with nursing supervision experience preferred.
• Minimum 1 year of home health or equivalent experience.
• Experience in case management, admissions/discharges, wound care, and documentation.

Responsibilities

• Coordinate case management, SOCs, admissions, recertifications and ROC.
• Lead field staff, supervise RN/LPN/HHA teams, and ensure care quality.
• Travel to clients within NW Ohio regions and maintain timely documentation.
