Job summary

Home Care Network, Inc. seeks an experienced RN Case Manager (RNCM) for a Part-Time or PRN leadership role across Northwest Ohio, with travel to multiple counties.

The RNCM will coordinate care, supervise nursing staff, and ensure high-quality home health services for ongoing patients. The role emphasizes case management, SOC, admissions, recertifications, and QA leadership, with flexible scheduling and mileage reimbursement.

Qualifications

• Minimum of 1 year of nursing supervisory experience.
• 1+ year of RN experience in home health or in-home care setting.
• CPR and First Aid certifications required (or obtainable before hire).
• Strong documentation skills and ability to supervise field staff.

Responsibilities

• Coordinate client assessments and care plans in home health settings.
• Supervise a team of RNs, LPNs, HHAs, and personal care aides.
• Document and communicate patient status and changes to the care team.
• Ensure safety and compliance with state and federal regulations (HIPAA).
• Travel to scheduled visits within service areas and counties.
• Promote professional development and training for staff.
