Job summary

Home Care Network, Inc. is seeking a Part-Time or PRN RN Case Manager/Recertification Nurse (RNCM) to oversee home health care services in Northwest Ohio. This field leadership role involves case management, SOC, admissions, recertifications, and supervision with travel to client sites.

Qualifications

• Minimum 1 year in case management including admissions/discharges, wound care, and transitions of care.
• Experience with OASIS and NON-OASIS assessments preferred.
• Nurse supervision or leadership experience is desirable.

Responsibilities

• Coordinate case management, SOC, admissions, recertifications, ROC, and clinical reviews.
• Provide staff supervision, training input, and ensure compliant documentation.
• Travel to client homes within NW Ohio and surrounding areas as required.
